 There are a series of forms you should fill out before arriving at our office for your first appointment. Filling them out beforehand will expedite your processing and it will allow you to arrive closer to your scheduled time than you should otherwise. There are five forms. They include your dental history, your medical history, your account information, your insurance information and our office policies. Please do not email them to us as we need your signature on each.
